Services: Information, referrals, peer support, mentoring, individual advocacy, systems change advocacy, youth and nursing home transition services, life skills training, and assistive technology equipment to help disabled individuals increase their independence in the community. Limited transportation services available. Serves Utah, Wasatch, Sanpete and Juab Counties. Intake process required to apply.
