After assessing your needs and completing a screening over the phone, you will be placed on the appropriate waiting list.
Once you are accepted onto a program, you will be assigned a case manager who acts as your advocate assisting in developing a care plan to support you through the aging process. Your case manager meets with you regularly to facilitate access to non-medical services to meet your needs.

Provides non-medical, in-home services to low-income seniors who are on Medicaid.
Provides non-medical, in-home services for older adults at risk of nursing home placement.
Provides Medicaid recipients residing in institutional settings with the option of receiving supportive services in their own home or in other community-based settings.
