These forms are only for RSVP volunteers who have been onboarded and given instructions to use these materials.

How to Apply for Mileage Reimbursement

  1. Fill out ACH form for direct deposit. Volunteers only need to fill it out once. Download with the link below.
  2. Download the mileage report. Fill out and return. This can be filled out as needed, monthly, or based on services.
  3. Submit forms:
    Email: rsvp@magutah.gov
    Mail/Drop off in office: MAG RSVP Program, 586 E 800 N, Orem, UT 84097
