Lynette Evans

Lynette Evans has been a Family Nurse Practitioner since 2012, primarily caring for patients with advanced heart failure and transplants for over a decade. But her story in caregiving started long before that—even as a teenager caring for her grandmother in her 90s. That experience planted a deep respect for seniors and has guided her ever since.

In 2024, she became a Certified Senior Transition Specialist and founded Senior Transition Partners, helping families navigate the difficult decision of a transition when home is no longer the safest place. Today, she continues that mission while stepping into a new role as a NP in mobile wound care, bringing her skills directly to patients at home.

As a caregiver at heart—and a mom to three children—Lynette understands the challenges families face. She combines medical expertise with compassion, offering guidance, peace of mind, and hope during life’s most important transitions.

Monica Boyer

Monica Boyer is a Registered Nurse with over 30 years of experience dedicated to serving the aging population. Her career began as a CNA, then LPN, and eventually Director of Nursing in a Skilled Nursing Facility. She has also worked extensively as a Hospice RN and Case Manager, supporting patients and families during some of life’s most tender moments. Currently, Monica serves with MAG Aging and Family Services, where she works with clients on the Aging Waiver Program and case manages participants in the Caregiver Program. She is also a certified instructor for Dealing with Dementia, and is passionate about educating older adults and their families about the aging process, in-home service options, and resources available in the community.
